Menopause / Schmenopause

Prepare for your GP appointment.

A calm, practical way to bring your story together. Use your tracker, add what matters most, then print, save or copy your summary.

Your information stays on this device. This page does not send your tracker entries anywhere. It helps you organise them for a conversation with a health professional.

Your tracker summary

We found the entries saved in this browser. You can include them in your appointment summary or leave them out.

Your appointment, in your words

You do not need to complete every section. The aim is to make the appointment easier, not to make it perfect.

For example: “I do not feel like myself and my sleep is affecting everything,” or “I want to talk about my changing periods and hot flushes.”

How is this affecting your life?

Tick anything that is true. It can help the GP understand the impact, not just the symptoms.

A rough answer is enough: for example, “about 18 months ago,” “after my hysterectomy,” or “since starting treatment.”

Leave blank if you do not know or it does not apply.

Only include what you are comfortable printing or sharing. You can also bring a photo of your medication list.

Questions you might want to ask

Choose any that feel useful. These are prompts, not medical advice.

Prepared . This is a record of what I have noticed and would like to discuss; it is not a medical assessment.

A quick safety note

Do not wait for a planned appointment-prep session if you have bleeding after 12 months without a period. Please arrange a GP appointment, even if it happened once or was only spotting. Unusual bleeding between periods or after sex should also be checked. Read NHS advice.

For urgent health advice, use the NHS service appropriate to where you are, including NHS 111 when needed.
